All parents are advised to read the Information for Parents Booklet that gives details of how applications are processed.

Declaration

I confirm that I have read and understand the supporting documents on the relevant admissions page on the Council Website.

I confirm that I am the legal guardian, holding parental responsibility for the child concerned, and that all the information included in this application is true to the best of my knowledge.

I acknowledge that Cardiff Council are permitted to withdraw any offer of a place for my child, if it is found that the information I declare on the application is knowingly falsified.

I understand that officers of the Pupil Admissions Team will be accessing data held by the Council’s Council Tax Department in order to validate that the address I have declared on this application is correct. Therefore consent will be assumed by the making of an on line application.

NB: If you are unwilling to give consent for this you should make a paper application for which original documents will be required.

Written recommendations from a medical consultant, a social worker or similar professional must be provided giving detailed reasons for the pupil’s admission to a particular school.

I understand that any additional information in support of my application must be sent to Pupil Admissions, Cardiff Council, County Hall, Atlantic Wharf, Cardiff CF10 4UW
